Paperwork Required to Adopt from Brazil

The collection of documents you gather for the Brazilian Adoption Officials is called your dossier. Listed below are the documents that need to be included in your dossier which is submitted to the Brazilian adoption officials if you adopt from Brazil.

Please do not begin gathering and/or preparing these documents until you receive our "Brazil Dossier Guide". Our guide provides detailed instructions and sample forms to ensure that all your documents are correct and acceptable to the Brazilian adoption officials. We will provide you our "Brazil Dossier Guide" immediately upon receiving your completed Application for Adoption & Adoption Services Agreement.

  1. Approval that is issued by the CIS after reviewing the I-800A and all supporting documents. *
  2. Home study. *
  3. Psychologist report. * (May be included in the home study).
  4. Photocopies of the adopting parent’s passport – photo and signature page. *
  5. Copy of your previous year's federal income tax return (does not require any schedules). *
  6. Marriage Certificate – if married. *
  7. Divorce Decree – if divorced and not remarried. *
  8. Birth Certificate. *
  9. Medical Form. *
  10. Local Criminal History Check. *
  11. Declaration of Adopting Parents. *
  12. Pictures of Adopting Parents and applicants' home – inside and out. *
  13. Certificate of Residence – this would include proof of home ownership or an affidavit from landlord as to your apartment rental or lease. *
  14. Declaration of an official verifying your State's Adoption Law – FTIA will work with you and the home study agency to complete this document. *
  15. Power of Attorney. *

* These documents must be notarized (or certified from an appropriate state court or agency), certified by the Secretary of State from the state the document originated, and authenticated by the appropriate Brazilian Consulate. Specific instructions for this procedure are given in the Brazil Dossier Guide.

Acknowledgement & Agreement

When accepting the referral of a child all adopting parents are required to sign an FTIA Acknowledgement & Agreement in addition to any acceptance paperwork submitted to the foreign country. This Acknowledgement & Agreement confirms your decision to complete the adoption of the child referred to you and reiterates the post placement requirements once you bring the child home.
